A 1095-C is a form employers use to report information about the health insurance they offered to employees. It’s part of the Affordable Care Act (ACA) requirements.

1095-C forms help employees when filing their taxes, as they indicate whether the employer offered them health coverage that meets ACA standards. The form is not required to prepare or submit your 2024 taxes.

The Paperwork Burden Reduction Act (H.R. 3797), passed recently, now provides some flexibility in terms of providing a copy of the applicable 1095 forms to employees.

The Affordable Care Act of 2010 requires that employers provide their employees with information regarding New York’s “Health Insurance Marketplace”.  The Marketplace opened on October 1, 2013 and began providing alternative access to health insurance coverage effective January 2014.  Employees are asked to review the documents below regarding this health insurance option.  Please note that:

• the Affordable Care Act requires that we inform you of your option to acquire health insurance through the Health Insurance Marketplace.
• the school district will continue to offer the same health insurance to its employees.  You do not need to do anything if you are happy with your current coverage.

On December 18, 2017, New York State Governor Andrew Cuomo signed into law an amendment of Section 159-b of the state Civil Service Law. The amendment entitles any public employee the right to paid, excused leave of absence from his or her duties for a sufficient period of time, not to exceed four hours, to undertake a screening for cancer of any kind.  The law had previously provided leave for public employees for only breast and prostate cancer screenings.

The employee may be required to provide satisfactory medical documentation to substantiate that their absence was for the purpose of screening for cancer.  Failure to provide appropriate documentation may result in the employee being charged a sick or personal day.

Leave for cancer screening is not cumulative and expires at the close of business on the last day of each calendar year.

So you need to take a leave of absence? There are many reasons a person needs to take some time away from work, and each has its own criteria or “rules” to adhere to.  Some leaves can be paid – but can only be paid if you have sick, personal or vacation time available.  Some are unpaid, but under certain circumstances your health benefits continue as though you were continuing to work.  Some require you to submit a letter requesting the leave, and depending on the circumstances, the district will either approve or deny the request.  Regardless of the type of leave, we ask that employees adhere to the guidelines outlined in your employment contract to ensure that your leave is taken without cause for concern, and that your wages and benefits are properly handled.  Of course, if you have questions, please feel free to contact us for assistance.

Procedures for Reconciling Paid Time Off

It is essential that procedures are followed to confirm the accuracy of paid time off requests/notifications.  Therefore designated individuals will retrieve information from the sub calling service system and nVision to verify individuals are submitting time off requests/notifications in nVision and the sub calling system.  In the event discrepancies are found, designated individuals will notify affected employees of the need to submit time off requests in nVision.  Employees who, on a recurring basis, fail to submit time off requests/notifications will be counseled accordingly.
